Appointments

• Providers focus on medication management when it’s determined to be the correct course of treatment.

• Individuals prescribed controlled substances (Adderall, Vyvanse, Ativan, Klonopin, etc.) will be required to have at least one in-person appointment to remain in compliance with the Drug Enforcement Administration (DEA) Ryan Haight Act.

• Though we provide accommodations, we don’t provide crisis management. In the event of an emergency or life-threatening situation during or outside of our normal business hours, please utilize 911 or go to the nearest emergency department.

Cancellation Policy

• Individuals are requested to provide us with at least a 24-hour notice of appointment cancellation or reschedule.

• There’s a no-show and late cancelation fee of $100 for individuals who cancel less than 24 hours before their scheduled appointment or fail to attend their visit.

• It’s crucial to show up no later than 10 minutes before the scheduled appointment time.

• Repeated missed appointments may lead to discharge from the practice.

PRESCRIPTIONS AND REFILLS

• In attempt to maximize each patient’s health and safety, we request that individuals allow a minimum of 48-72 hours for prescription refills to be sent upon request.

• Controlled substances require a monthly refill and cannot be dispensed earlier than the given timeframe.

• Individuals are expected to send in a refill request, if the pharmacy doesn’t do so, to avoid any discrepancies in treatment.

DISABILITY, FMLA, ESA, AND FORMS

• Each request will be considered on a case-by-case. However, it will be required that the individual has established care with the practice for a minimum of 3 months (and at the discretion of the provider).

• An appointment must be scheduled to discuss and assess the need to file for disability.

• Any forms or letters that need to be reviewed outside of the appointment will be subject to a fee of $50-150 based on complexity.

• A release of information (ROI) must be signed in order for records to be shared with other health care providers.
